• Value Added Service | Diabetes Management

  • Dear Cook Children’s Health Plan Member,

    Cook Children’s Health Plan (CCHP) has a value-added service for our STAR Members only. You can receive up to $150 in gift cards each year.

    *Date of service must be within the current state fiscal year (September 1, 2026 - August 31, 2027)

  • Cook Children's Health Plan Members must be:

    aOn the STAR Medicaid plan

    		 a Diagnosed with Diabetes I or II
    a Ages 18 to 20 years old a NOT diagnosed with pregnancy related diabetes only

    You can get $25 for completing each of the items below. You can only claim each item once per year.

    1. Complete a Diabetes Health Risk Assessment with CCHP staff by telephone.
    2. Get your Hemoglobin A1c (HbA1c) tested at your primary care provider's (PCP) or endocrinologist's office.
    3. Keep your HbA1c level under 8%.
    4. Complete both eGFR (estimated glomerular filtration ratio) under uACR (urine albumin-creatinine ratio) testing at your primary care provider's office or endocrinologist's office.
    5. Keep your blood pressure (BP) under 140/90. BP reading must be taken at a provider's office. Blood pressure readings taken when you are in the hospital or having a special test in a doctor's office , clinic or hospital do not count. 
    6. Get a full diabetic eye exam with an optometrist or ophthalmologist. 

  • You can send this form after you complete each one or wait and send it after all six are done. This will keep us from duplicating rewards. Remember, you can only receive one $25 gift card per item, per year.

    Each item can take up to 90 days to verify, so thank you for your patience.
