Dear Cook Children’s Health Plan Member,

Cook Children’s Health Plan would like to send one $30 gift card per school year to Members ages 12 to 18, for participating in one extracurricular activity, school sport or school club. Proof of participation is required.

*Date of service must be within the current state fiscal year (September 1, 2026 - August 31, 2027)
