    Cook Children’s Health Plan would like to send $50 per year for pest control services for Members with a chronic respiratory condition enrolled in the Asthma Disease Management program and/or in Service Coordination.

     

    *Date of service must be within the current state fiscal year (September 1, 2026 -August 31, 2027).
