Value Added Service | Utility Set Up Fee Assistance

Dear Member of Cook Children’s Health Plan,

Cook Children’s Health Plan would like to send you a $50 gift card. This is to be used to cover initial set up fees for your utility services (water, gas or electricity).

This is a bi-annual benefit. There is a limit of one gift card per household, per family. Each member within your household must complete the Cook Children’s Initial Health Needs Screening within 30 days of enrollment.

 

*Date of service must be within the current state fiscal year (September 1, 2026 - August 31, 2027)
